Planning and Support

The HeathCliff team provides innovative, individualised support and care. We help develop a care plan to match your requirements, goals and needs. We go above and beyond normal service provision expectations and build partnerships with all stakeholders to ensure a collaborative approach is undertaken to best support our participants. HeathCliff Support maintains a communication platform for reporting to all key stakeholders and values the input of all key stakeholders into the planning implementation and review processes.


Supported Independent Living
As a specialist supplier of Supported Independent Living (SIL) services, HeathCliff Support maintains the highest level of trained staff who maintain the highest level of care. We provide independent living support in many different ways throughout Brisbane and the Sunshine Coast. Whether it's your own home, or within one of the many private homes we have access to, our goal is for you to be comfortable in a home that supports your desired lifestyle. Our independent or shared house model is designed to nurture, engage and teach our participants skills to help them live the life they choose. HeathCliff Support works closely with all stakeholders and family to ensure that the focus remains on meeting the individual needs of our clients.
Short Term Accommodation
and Respite
​
Short Term Accommodation (STA), sometimes referred to as respite, is funded through your core supports budget. STA is different from a holiday or a recreational activity. Participants use STA as part of their NDIS capacity building plan in a variety of ways. This will be dependent upon your goals.
Respite care is designed to give both you and your informal supports time to rest and recharge, at the same time providing opportunities to focus on capacity building in a supported environment.
Participants always love to visit HeathCliff’s farm house situated in the picturesque hilltop Mapleton region. Well known for its pristine nature walks, waterfalls and abundant flora and fauna, this 15 acre farm stay will not disappoint. With wheelchair access, toilet and showering aids, it is the perfect all-inclusive short term and respite accommodation solution. The HeathClifff team pride themselves on making your stay a safe, relaxing, enjoyable and memorable one.


Community Participation

HeathCliff Support have a strong track record changing the community access component of all clients. We believe that participants’ social lives are important to their wellbeing and quality of life. Community access and engagement enables opportunities for connection and growth. We support our participants to access their community and engage in opportunities to achieve gainful employment, social and recreational activities. We deliver our services and types of activities based on the needs and interests of those whom we support.
